If you’re a man or you know a man who may be interested in looking more athletic, chiselled and looking fresher and younger then read on as I describe the top three ways I use dermal fillers to help men look fantastic when they attend me at Castleknock Cosmetic Clinic, Dublin.

So what are fillers?

Fillers are naturally occurring substances that are injected to help reshape the face without the need for surgery. Popular hyaluronic acid dermal fillers include Restylane, Juvéderm and Belotero. Long lasting dermal fillers include Radiesse and Ellansé.

STRONG CHIN

One of the key areas in treating men is the chin. Strong chins are synonymous with strength and confidence. We look to see strong chins in leaders in politics and in business. Examples can be seen in athletes and hero’s we admire. In Hollywood a strong chin can mean the difference between a supporting actor and a leading man. Fillers can be used to add one or two centimetres of projection to the chin. Guys’, adding one or two centimetre projection to your chin is like adding one or two inches to your height. Chin can make you look more athletic, can give you more of an alpha male appearance and it can make you look more chiselled.

UNDEREYE

The second great place to treat me with fillers is under the eyes. Fillers are fantastic in removing dark circles and bags and this can make us fresher and more youthful without the need for surgery. People who have dark circles and pigmentation under the eye are helped with fillers camouflage and restore fullness in the area making them look fresher and more youthful.

MASCULINISATION

The third treatment that’s great with fillers in men is full face masculinisation treatment. In this masculinisation treatment often small changes to key areas of the male face making the entire face looking more balanced, proportioned and better. In masculinisation treatment of the full face I target the undereyes, nose, cheeks and chin. The treatments are administered over a number of sessions and small changes can often make great impact.

So in summary, fillers can be used in men to non-surgically augment the chin making it more chiselled more athletic. They can be used to hide circles under the eyes making men look fresher and younger. What is Vaser Lipo?

You’ll hear about different types of liposuction; VASER Lipo, SmartLipo, Sculpting, CoolLipo, TrickleLipo mentioned and wonder what’s the difference? They are alternative methods of performing fat removal from targeted areas of the body. They differ in how they remove the unwanted fat and how they treat the adjoining tissue and skin. VASER lipo utilises ultrasound vibrations to protect important blood vessels and lymphatic channels while the heat generated from the repeated vibrations heats the skin for optimum contraction. VASER lipo is done under local anaesthesia where the treated area is injected with special fluid that will hydrate the area and shrink the blood vessels so they are not damaged.

So who is a candidate for VASER Lipo?

The better candidate is a person who has areas that are resistant to diet and exercise. Areas where liposuction is originally intended to be done are the hips (saddlebags), the lower tummy, inner or outer thighs. An example of the ideal candidate I see in my office at Castleknock Cosmetic Clinic Dublin is a tall lady who comes in and has fullness in the outer thighs, the so called saddlebag deformity. If you’re globally overweight and have a BMI index of more than 32-34 you really shouldn’t get liposuction. You should diet and exercise and get things reduced and then you can consider what needs to be done, a tummy tuck, a thigh lift etc. When I consider liposuction for an individual, I look at the quality of the skin. Skin elasticity is key when performing liposuction. The better the elasticity the greater the liposuction results. Young elastic skin is waiting to snap back and hold the treated area in smooth contour. Elastic skin is very forgiving even if we don’t do a great job in removing the fat evenly. Older skin or damaged skin with a lot of stretch marks has no elasticity. When the fat is taken out, it just folds irregularly and we get folds and divots and waviness. This is when VASER lipo’s ability to heat and shrink the skin shows to be more superior than other methods. Although it contracts the skin considerably with VASER technology, it may not do a perfect job and some contour irregularities will remain.For some people they don’t care about some contour irregularities as long as they look better in their clothes. As long as we understand that ahead of time I’m perfectly willing and able to do VASER lipo on that person. It’s all about expectations. If you’re expecting a nice tight smooth contour to your skin and you got much damaged skin by having stretch marks or having older skin that doesn’t have elasticity don’t be surprised if you have waviness afterwards. But in some cases that’ll be okay. Sounds like you’re the right fit ?

When I started performing non-surgical nose reshaping 10 years ago nobody was doing non-surgical rhinoplasty then. It was unheard of then and most plastic surgeons frowned upon it. They didn’t want anything injected into patient noses for fear of product migration or infection. But over the last decade of experience I’ve come to realise that if choosing the right candidate and the right dermal filler, I can get really nice results in a significant number of my patients. So I choose and select suitable candidates for the non-surgical nose reshaping procedure properly. The same rhinoplasty aesthetics and goals of treatment have to apply to the non-surgical procedure.

When I assess my patients I pay special attention to the aesthetics of the nose and it impact on the patients overall look and appearance.  We can’t just put any filler in the nose! Before performing the procedure I pay attention to the tip projection, bony prominence and axis rotation as well as the overall slope ratio. I use digital imaging on almost all patients presenting for non-surgical rhinoplasty.

One suitable patient is who present with a small dorsal hump. If their bony prominence is surrounded by depression above and below it, then I could I place an amount of dermal filler below and above the bump. It’s important to try and not go for perfect straightness. Favourite start up dermal filler is hyaluronic acid (HA) based dermal filler.

One reason I like to use hyaluronic acid dermal fillers for non-surgical nose reshaping is that they are safer for my patients. For starters HA dermal fillers are dissolvable and if the patient didn’t desire the aesthetic correction results after their nose job, it can be taken away safely. HA fillers are also a clear soft gels and not permanent. Some people come asking for silicone or Artefill as permanent fillers, but these products don’t belong in the nose. Permanent fillers give rise to permanent problems. Great new FDA approved dermal fillers bridging the gap between longevity and safety is the likes of Radiesse and Ellansé. Ellansé is ultimately degradable and lasts for up to four years.

We are all born with genetically predetermined body shape. When your body shape is awkward, no matter how much you diet or exercise you can still have stubborn pockets of fat that won’t go away. In this scenario surgical intervention with VASER lipo might be the answer for you?

Sometimes you don’t have to be fat to have fat!

In some people and often times is women, they have a pear shaped body. They are petite up top and slender in their waist but their bottom and thighs just really expand. It’s unfortunate for these women who tell me they lose a few pounds and it comes out of their face, breasts or arms but not their problem areas, the saddlebags. For these women VASER lipo is wonderful. I can really contour their thighs and buttocks and get them in balance again.

I have been doing liposuction since the late nineties and liposuction has come a long way. VASER lipo uses smaller instruments, done under local numbing and focuses on certain areas while being minimally-invasive. VASER lipo promises shorter recovery down time and better results.

VASER lipo is done under local anaesthesia. During the procedure which is performed in our surgical room at Castleknock Cosmetic Clinic, Dublin, I inject tumescent fluid in. The fluid numbs the area and shrinks local blood vessels. The fluid also hydrates the area and minimises bruising and swelling. The patient is awake during the whole procedure and they walk in and they walk out in a couple of hours later. Because they are awake we can ask them to turn and move into different positions during the surgery minimising lifting and handling.

What’s nice about VASER lipo is that it uses ultrasound vibrations to break down the fat. By doing so it protects from damaging blood vessels, connective tissues and lymphatic channels so it produces more tissue tightening afterwards. Vibration is also associated with heat generation. This heat is directed to the under surface of the skin promoting impressive post-operative tissue contraction.

Many patients come to me saying they hate their saddle bags. They say saddle bags add to their pear shaped appearance and although they exercise regularly, these annoying saddle bags don’t seem to go away. They would have tried everything they could have thought of to tackle their problem areas without any luck.  Ideal candidate is a fit person who is conscious of their diet and has trouble saddlebag area that they cannot eliminate with regular diet and exercise. VASER lipo is not a weight loss treatment. In the VASER lipo procedure we are really sculpting the body and changing the contours.

Brave enough men may have gone to their doctor who may not even know what these flesh coloured dome topped bumps are or told them they were perfectly normal and just forget about them and live with it? Even though PPP don’t have great physical impact they certainly affect the individual at a self-confidence and psychological level. Will they ever go away? No. They even become more visible in the erect state, how embarrassing!

PPP are usually noticed at a pretty young age, around puberty. Many affected men live with the condition and keep it like a “secret” ? Many men I see have developed serious psychological problems because of the pearly penile papules. For fear of being ridiculed or worry their sexual partner many men suffering PPP refuse intimate interaction with others and avoid sexual encounters.

The condition is one of the most common problems men have in the genital area. These PPP are called Pearly Penile Papules and they occur in up to 30% of men. Some men have more papules than others but usually they range between 150 and 250 pimple like skin tag type protrusions around the rim of the penis. We don’t know what the cause of PPP is but one thing for certain, these pearly penile papules are not contagious and in no way related to STD or genital warts.

I treat those embarrassing pimples down-there by using either SWD short wave diathermy or carbon dioxide laser. These treatments essentially destroy or vaporise the pesky little papules. Both procedures are completely painless. I administer the SWD or CO2 Laser treatments after administering local anaesthesia block on the patient’s penis area. Once treated these annoying papules are gone once and for all with a confidence rating of over 90%. Two to three weeks later it is recovered and ready for action!

Dermal fillers are on the rise, but with increasing popularity and people opting for cheaper and often ill-experienced administrators, comes an increase of risks and side-effects.
Here at Castleknock Cosmetic Clinic, we are seeing more and more unfortunate individuals presenting to us with lumpy and irregular lips following a bad lip augmentation elsewhere. It’s not unusual for patients to have contour irregularities of the facial skin in the immediate period following dermal filler injections. In the vast majority of patients this is usually related to either bruising or swelling and may take a week or so to resolve. With good injection technique, long term bumps should be rare. Lumps and bumps only occur in less than 5% of treatments and should be very small and hardly noticeable when they do occur.

Unfortunately, in unskilled hands these lumps, bumps and areas of hardness may be related to uneven injection of the dermal filler or superficial injection of a filler intended for deeper application. Although hardness can happen with any filler, more commonly they are caused by too much of the filler being injected at one point in the skin. Hyaluronic acid dermal fillers tend to absorb water which can accentuate the problem, specially in the lips where they’re noticed as lentil sized beads.

If the bumps persist, they can be treated. In some cases gentle massage will address the problem. In other cases removal of the offending dermal filler will be requires and thats when I see patients presenting to me for correction. Removal of dermal fillers irregularities can be accomplished by either manual extraction using a specialised instrument or by using the enzyme hyaluronidase to dissolve the material.

In Summary:

Once the treatment settles lumpiness should not be felt. Persistent irregularities can be treated successfully

What to do if you notice lumps after my dermal filler injections?

Infection needs to be ruled out. Contact your treating doctor to assess if antibiotics and other treatments may be required.

If you can only feel the lumps and not see them, then don’t panic, because simple massage should take care of the irregularities.

• Physical removal by a specialist extraction needle

With the increasing popularity of image centered social media, non-surgical chin definition procedures have grown in popularity. That selfie spoiling weak chin can now be fixed in a few minutes with a sophisticated chin augmentation procedure at Castleknock Cosmetic Clinic Dublin.

Cosmetic surgeons and doctors had no short-term option for treating weak chins until specialty lifting dermal fillers came along filling the gap between permanent implants and short lasting generic fillers. In the past, the only option was a one-size-fits-all chin implant. Chin implants weren’t ideal options due to lack of individual customization, you see; every chin is different!. Now we have the ability to shape someone’s chin based on and in harmony with the rest of their unique facial features.

A person with a stronger chin is perceived to be stronger, smarter, and more of a determined type of person. It’s no coincidence that super heroes and most successful fashion models have this distinctive feature.

When patients come to see if they’re a candidate for a chin job, I often take a side profile photo and draw a line from the innermost corner of their eye past their nose, and down toward their chin. If that line continues to be straight, then their chin is proportionate to their face. If it goes inward at an angle toward their neck, then they have a recessive chin. This is where specialist dermal fillers can help.

For patients who are hesitant about the non-surgical procedure, I offer them a salt water trial where I inject the saline water where I would normally put the fillers so they can see the results. The liquid only stays in the area for about five minutes and gives them a great idea of the level of augmentation they desire. Once the patient decides if he or she wants to move forward, they have a few options: If you’re looking for something more temporary, I would recommend going with hyaluronic acid options like Belotero or Juvéderm. These will last four to six months and cost around €450 to €650 depending on how many syringes are needed to build out the chin. If you’re looking for results to last longer up then I will recommend Perlane or Radiesse which last about a year. For more durable results, Ellansé is on a league of its own lasting 3 or 4 years at a time. Ellansé is made up of microscopic bio-stimulator beads which is why it lasts so long. Since the results are so immediate, if you don’t like what you see, the hyaluronic acid fillers can be dissolved away with an injected enzyme called hyaluronidase.

At Castleknock Cosmetic Clinic Dublin we love fixing lip lines. Lip lines are one of the most common complaints we hear our mature patients complain about. Mature patients want their smoker’s lines corrected but yet they worry about having large lips or looking like they have their lips enlarged!

These vertical lip lines are commonly referred to as “smoker’s lines” or “lipstick bleed lines”. The annoying lip line wrinkles are caused by a variety of factors, including age, UV light exposure, and genetic predisposition, repetitive lip movement lifestyle habits like smoking or drinking from a straw.

For the first time in Ireland we are introducing a new concept of dermal filler using a homogeneous form of the patient’s own fat, called nano fat fill. Nano fat fill is a totally new fat transfer concept. I would like to thank IBAM for introducing this innovative treatment concept to me.

Nano fat transfer is basically like a filler. Just like lipofilling, I harvest the fat from the patient, from donor areas like the tummy or thigh and I process it to make it liquid in form.or introducing this innovative treatment concept to me. I’ve been performing it for the last few months with brilliant results.

This nano fat is used to improve skin quality and it is especially useful for under eye dark circles and facial loss of volume. In undereye circles the problems are the poor quality of the undereye skin as well as hollowness and the presence of pigmentation. The nano fat fill is used to produce a natural volume restoration effect by filling the unwanted hollowness as well as tightening and improving the skin tone quality. Nano fat fill’s party piece is to significantly improve tone and dark circles pigmentation.

Nano fat fill has a lot of stem cells that have stimulating growth factors in them. It is well proven that stem cells taken from harvested fat are more superior in quality than those taken from other entities like PRP. During nano fat fill we collect and then purify the fat to concentrate and then extract stem cells. Only then we utilize the liquid consistency and stem cell rich purified fat collection for injection.

The nano fat fill system modifies the size of the fat particles to a per-dialled in desired diameter. Coarse for volumising effect and fine for stem cell repair strategies. The desired thickness nano fat fill then is injected under the eyes in a layered fashion. Very fine particles are used for their stem cell rich properties to improve on tone and pigmentation. Fine particles are injected to thin skin to improve thickness of crêpe skin texture. Larger particles are used to volumise hollow or gaunt facial regions like cheeks, facial folds and lips. For areas other than the face Nano fat transfer fill is used in the decolletage and the hands.

Most people with the condition notice their arms getting large when in their late teen years. Much recall sitting in class when the guy behind me ask “why my arms are so big”… they join a gym that and begin trying to work on their upper body. Although they could drop a size or two, their arms remain large, and stay big! They suffer from a condition known as ‘bat wings’, the large fatty wing-like feature that hangs below your arm. The extra fat and associated loose skin around the upper arm wiggles and wobbles in an aesthetically unpleasant manner and can cause a cosmetically distasteful thickness of the upper arm. Sufferers find bat wings interfere with wearing short sleeves and tank tops making them look unattractive.

People with bat wings suffer from excess fat tissue of the upper arm. Bat wing arm is a relatively common condition. Excess fat of the upper arms is usually a hereditary condition, although bat wings can also be caused by overweight and obesity. The result can be unsightly sagging and wobbling that can be mild or very severe in nature. My patients also refer to their large sagging upper arms as “bingo wings”.

Available Treatments

Patients can have their excess fat and loose skin of the upper arm treated through a variety of procedures. It is important to achieve a reasonable and consistent body weight that can be maintained and then one of several options can be used to improve the appearance of bat wings. The most common and effective treatment for bat wings is VASER liposuction of the upper arm. During bat wing VASER Lipo, a small incision made around the elbow region and another hidden at the top of the upper arm. The tiny 3mm incisions are used as an access port for the VASER probes. The VASER probe transmits ultrasound energy that both disrupts the fat cells and also itches the under surface of the skin. That itching process generates controlled heating of the skin. It is that process of itching and heating of the underlying surface of the skin what causes future contraction of the skin. Using a small cannula the loose fat is then suctioned out. A cannula is a hollow tube connected to suction device and a collection canister. The loose fat is then removed from the arm and the improvement of the bat wing relies on skin retraction and elasticity. VASER lipo for bat bingo wings is a once off procedure.

Another popular way to treat bat wings for those individuals that have small enough fat is by fat freezing.  Fat freezing is a medically controlled process during which a suction device that cools the skin to below freezing is attached to the bulk of the bat wing. The fat freezing device is left in for about 30 minutes and causes the fat tissue to freeze. The frozen fat cells perish through a process known as apoptosis. Over the next few weeks the fat cells are removed by the body leaving the arm thinner. Repeated treatments are required to achieve good results.

Patients who undergo VASER lipo or upper arm fat freezing generally will not have a return of this aesthetically displeasing bat wing condition, as long as careful weight maintenance is observed!
