Most men who ring the clinic about gynaecomastia have already read two opposite things online. One site says liposuction sorts it. Another says you can’t suck out gland, so only surgery works. Both are half right. The honest answer wither VASER vs surgery for gynaecomastia depends on what’s actually sitting under your nipple, and I can usually tell that within a few minutes of examining you.

What you’re trying to remove

Three different things can make a male chest look full, and each needs a different tool.

Fat is soft and spreads across the whole chest. Gland is the firm, rubbery disc right under the nipple, and it’s what makes the nipple look puffy. Skin is the third, and it only matters when the chest has been large for years or after a big weight loss, because the skin stretches and doesn’t shrink back on its own.

Most men I see have fat and gland together. Far fewer have real skin excess.

Traditional gland excision

The classic operation cuts the gland out through an incision along the lower edge of the areola. It’s usually done under general anaesthetic in a hospital theatre. Its strength is that the surgeon can see the gland and remove a dense lump in one piece.

The weakness is what’s left around it. If the gland comes out on its own and the fat around it stays, the chest can end up with a dip right under the nipple, sometimes called a saucer deformity. You also have a scar at the areola, a general anaesthetic and a hospital stay.

VASER liposuction for gynaecomastia

VASER uses ultrasound to break up both the fat and the tougher gland before I remove them through openings a few millimetres long. It’s done under local anaesthetic, and you go home the same day.

The big advantage is shape. Because I’m treating the whole chest, I can blend the edges and avoid that dip under the nipple. The ultrasound also helps the skin tighten as it heals.

VASER has two limits. A very fibrous core of gland can resist it, and it can’t remove loose skin.

Why I often combine the two

In my clinic, VASER does most of the work. If a firm fibrous core is still there afterwards, I remove it through a small incision at the edge of the areola, where the colour change helps hide the scar.

Because VASER has already cleared most of the tissue, that incision is smaller than in a classic open operation. You get the contour of liposuction and the completeness of excision. I’ll tell you at your planning consultation if I think you’ll need it, so it’s agreed before the day.

What about loose skin?

Some men worry that the skin will sag once the tissue is gone. With mild to moderate gynaecomastia, I add Renuvion skin tightening alongside VASER where the skin needs extra help. Renuvion uses helium plasma under the skin to make it contract as it heals. In my experience, it means a skin-cutting operation is seldom needed.

Where the skin genuinely hangs in folds, usually after a very large weight loss or years of severe gynaecomastia, a skin reduction operation with longer scars may still be the better choice. I’ll be straight with you at your consultation about which group you’re in.

Which option fits your chest?

As a rough guide:

  • Mostly soft fat, no firm lump: VASER on its own is usually enough.
  • Puffy nipples with a firm disc underneath: VASER, sometimes with a small excision at the areola.
  • Mild to moderate loose skin: VASER with Renuvion skin tightening.
  • Severe skin that hangs in folds: a skin reduction operation may still be needed.

You can’t really sort yourself into one of these from a mirror. The pinch test helps, but I’d rather examine you than have you guess.
